Our Childcare Agreement

All families are required to complete this agreement prior to the first day of care. The Childcare Agreement is supplemented by our Handbook. 

Childcare Agreement

 

I(we)                                                                        &                                                              agree to the terms, conditions and policies as outlined in this document and the Good Earth Childcare Handbook, and enter into a childcare agreement on                       (date) with Anna Swank (provider) for the care of my(our) child(ren).

Rates: Completed based on each individual family's childcare schedule.

Deposit: A deposit in the amount of $100 per family is due upon enrollment. After one calendar year of care, the deposit will be reimbursed upon termination, provided all provisions in this Agreement have been met.

Payment: Payment may be made by cash, check payable to Anna Swank, or credit/debit card (subject to a 5% processing fee). Contracted Fees are due in advance on the last day of care the preceding week, and are due each week regardless of attendance. Drop-In care is calculated on a daily basis and accumulated throughout the week. Payment for Drop-In care is due at pick-up on the last day of care in the current week.

Termination: Parent must provide a minimum 30-days notice of termination of this contract.

Holidays: Provider is closed on the following holidays: New Years Day, Good Friday, Memorial Day, Independence Day, Labor Day, Halloween/Blacklick Beggar’s Night (close @4pm), Thanksgiving Day & the day following, Christmas Eve, and Christmas Day. In the event a holiday falls on a Saturday, provider will be closed the preceding Friday. In the event a holiday falls on a Sunday, provider will be closed the following Monday. No fee adjustments or refunds will be made for holiday closures.

Vacations: Provider is closed for two one-week vacations each year. 60-days notice will be provided to all parents prior to vacation closures. Contracted parents may take these weeks unpaid, or they may reserve their unpaid weeks (up to two per calendar year) for use at another time, with 30-days notice to provider.

Sick & Personal Days: Provider is allotted up to 5 paid sick/personal days per calendar year. 10-days notice will be provided for personal days. In the event of provider illness, notice will be given by 6am the morning of care. Notice is typically given as a group text message to all parents. No refunds or adjustments will be made for the first 5 sick/personal closures per calendar year. Closings for sick/personal days beyond 5 per calendar year will be considered unpaid, and families will receive a pro-rated credit towards childcare fees the week immediately following the unpaid closure. No refunds will be given for unpaid sick/personal days.

Emergency Closures: Provider may close early or for the entire day in the event of emergencies such as, but not limited to: power outages, water outages, or extreme weather conditions. Provider is closed when Franklin County is under a Level 3 Snow Emergency. Provider may also close in the event of a Level 1 or Level 2 Snow Emergency if there is significant concern that conditions will worsen throughout the day. No refunds or adjustments will be made for the first 3 emergency closures per calendar year. Closings beyond 3 per calendar year will be considered unpaid, and families will receive a pro-rated credit towards childcare fees for the week immediately following the unpaid closure. No refunds will be given for unpaid emergency closures.

Early Drop-Off/Late Pick-Up: Drop-off prior to 7:30am and/or pick-up after 5:30pm is subject to availability, and must be arranged at least 24 hours in advance. The rate for early drop-off/late pick-up is $10 per hour for one child, or $15 per hour for two or more children. There is a 1-hour minimum fee, and rates are charged per half-hour thereafter. Without prior arrangements, the early drop-off/late pick-up fee is $20 for the first 15 minutes, and $1 for each additional minute for drop-off prior to 7:30am or pick-up after 5:30pm according to the provider’s clock. Early drop-off/late pick-up fees are due the same day of service.

Additional Fees:

Late Payment Fee: $5 per calendar day for each day payment is late beginning the second calendar day following payment due date. The late payment fee is assessed per calendar day, which includes weekends and holidays.

Returned Check Fee: $35 plus any bank fees incurred by provider. The returned check fee and payment of the returned check is due in cash on the second calendar day following notice of the return. On the third calendar day after return notice has been provided, late fees retroactive to the original payment due date will be assessed, and will continue to accrue until payment is made in full. If two (2) checks are returned unpaid for any reason, all future payments must be made by cash or credit/debit card (subject to a 5% processing fee).

Supplies & Belongings:

Diapers, Pull-Ups & Wipes: Parent is required to provide a one-month supply of diapers/pull-ups and wipes each month. Alternately, parent may opt to pay a Supply Fee of $25 per month for the provider to supply diapers/pull-ups and wipes. The supply fee or supplies are due by the 3rd of each month.

Extra Clothing: Parent must supply one seasonally appropriate complete change of clothing for their child(ren). Extra clothing will be left in the provider’s possession until it is used or swapped out at the change of seasons.

Proper Attire: Shoes must be slip-on or velcro. Athletic-style sandals with a full back are permitted. Other sandals and Crocs are prohibited for children under 5. Clothing must be weather appropriate and suitable for a childcare environment. Attire should allow the child to be outside comfortably for up to 30 minutes in colder months, and several hours in warmer months. If desired, parent may supply hat and gloves in the winter and a swimsuit and towel in the summer to be left in the provider’s possession.

Quiet Time: Parents must supply two blankets and a small pillow for their child(ren)’s use during quiet time each day. A pacifier or other security item may also be provided if the child requires it. Quiet Time supplies must be left in the provider’s possession, and will be laundered by the provider each week or more frequently if needed.

Illness:

Communicable Diseases: Illnesses such as conjunctivitis (pink eye), strep throat, impetigo, infectious diarrhea, chicken pox, scarlet fever, and other highly contagious, serious illnesses require that your child remain home or be removed from childcare as soon as possible. The child may return to care when they are no longer contagious. Provider may require a doctor’s note prior to allowing the child to return to care.

Fevers: Fevers must be controlled by over-the-counter medication at home before the child arrives for childcare. Children who arrive with a fever will not be admitted for care. In the event fever develops while the child is in provider’s care, provider will notify parent and determine the best course of action.

Vomiting & Diarrhea: A child who is vomiting and/or has diarrhea will not be admitted for care until they are able to hold food down and/or their bowel movements have returned to normal. Children who have not eaten since their last bout of vomiting or diarrhea will not be admitted for care. In the event vomiting or diarrhea develops while the child is in provider’s care, provider will notify parent and determine the best course of action.

Lice: Children will not be admitted for care until they are completely nit-free, which typically takes up to 10 days. Provider offers chemical-free lice treatment for $75. Fee includes the first treatment, instructions and supplies to continue treatment at home.

Revisions to this Childcare Agreement may be made from time to time, and will be communicated in writing. Lack of enforcement of any policy at any time  does not indicate that the particular policy is no longer in effect. By signing below, I/we affirm that I/we agree to all provisions contained herein.
